Industrial Accident: Flash Fire in Pneumatic Control

Industrial Accident:  Worker was burned in a flash fire while servicing a natural gas (methane) operated pneumatic control panel. The incident occurred on a normally unmanned offshore oil and gas platform during a routine maintenance procedure. Forensic engineering included analysis of the control panel design and fabrication, maintenance service procedures, and static buildup on plastic tubing used in a pneumatic control circuit. The forensic analysis was used to evaluate the likely ignition source (spark) that caused a flash fire fueled by natural gas normally and abnormally released by pneumatic control devices.

Gas operated control panel

A pneumatic control panel on an offshore platoform can be operated with compressed air or natural gas (methane). Natural gas is often used becasue it is readily available on an oil and gas production facility, both on land sites and offshore platform. The pneumatic panel design and fabrication is similar for both gas and air operation. Material selection and venting are key areas of concern. Panels operated by natural gas methane must use materials compatible with the gas and condensate oil that will come in contact with the pneumatic control devices and circuits. The design and fabrication of related electrical and electronic devices and circuits must also be considered if natural gas methane will be within the panel vacinity or enclosure.

Static Buildup

An electrical spark can occur due to static buildup on surfaces of some materials. Of concern in gas operated control panels is buildup of electrical static on plastic tubing used in pneumatic control circuits. The buidlup is caused by gas flow through the plastic tubing. This buildup can be safely discharged by bonding the tubing to earth ground. The bond to ground is necessary to prevent buildup that can cause a spark capable of igniting a narural gas methane atmosphere.

The forensic engineering in this matter included examination of the control panel design and fabrication that included plastic tubing and connection fittings. No testing was done on the materials. Relevant data sheets were used as a reference to buildup poential due to gas flow through the plastic tubing. The conclusion was that the flow rate was not sufficient to cause adequate buildup in a moist offshore atomsphere.

Forensic Engineer

A forensic engineer performs analysis and reporting on technhical matters that are typically being pricessed through some form of legal matter. However, a legal environment isn't required for a forensic examination. The analysis may be performed merely to determine the cause of a specific event or condition. For example, a forensic examination may be made on a control system to determine why an accident occured, or why a system did not perform as expected. The forensic analysis may be of software code such as ladder lofic in a PLC, or it may involve hard wired relay logic, electrical controls, power distribution, or instrumentation. Forensic engineering is therefore useful in a variety of situations regardless of the legal entanglement.

Industrial Equipment

Typical equipment includes programmable logic controller PLC, distrubited control system DCS, and electric relay logic. PLC systems use ladder logic for most operations, while a DCS will often use function block programming. The concepts of PLC and DCS have merged into a unified control platform based on open architecture interfaces. The use if ladder logic is widespread due to its earlier application to relay logic circuits.

Engineer for Machine Accident

A certified systems integrator is someone who can be an expert engineer for automatic control. The systems integration involves combining multiple equipment and techology into a single control system. This involves design, programming, fabrication, testing installation, and maintenance.
